THERE WILL BE NO POSTPONEMENTS FOR THE TERM DATES OF NOVEMBER 23RD OR NOVEMBER 30TH. ALL POSTPONEMENTS FOR THESE TWO WEEKS MUST HAVE BEEN MADE WITHIN 10 DAYS OF RECEIVING YOUR SUMMONS AND PRIOR TO TODAY'S DATE.

INSTRUCTIONS

If you have received a qualification questionnaire please take the time to read all of the instructions. The form is not a summons for jury service. This form is used to determine whether you meet the qualifications for service as a juror only. If you are deemed qualified to serve as a juror, you may receive a summons to appear for jury service at a later date. There are no longer any automatic exemptions from jury service. All requests for an excuse from service should be raised at the time you actually receive the juror summons.

If you check any of the boxes in the blue area for questions number 6, 7, 8, 9 or 10 you must provide the appropriate proof as indicated.

General Information
Telephone Standby:
All trial jurors are on call with their qualification/summons and are to report to the courthouse only when instructed to do so by means of a recorded message. The telephone number is 1-800-449-2819.

Parking:
Free parking is not available. Numerous lots are within easy walking distance of the courthouse. Metro bus/rail schedules can be obtained by calling 716-855-7211.

Absence/Emergency:
If an illness or emergency prevents you from reporting, please call our office at 716-845-2555 beginning at 8:00 a.m. on the day you are to report. Court closings due to snow or storms are announced on local radio and television stations.

Court Hours:
Courts normally operate from 9:00 a.m. to 5:00 pm, Monday through Friday.

Court Security:
Entry to the courthouse begins at 8:30 a.m. at both the 92 Franklin Street entrance and the 25 Delaware Ave. entrance. Weapons of any kind and all sharp or dangerous instruments, such as scissors and pocket knives, are prohibited in the court buildings. Jurors are not exempt from magnetometer screening and your cooperation and patience is appreciated.

Verification of Attendance:
An attendance verification form is available to all jurors at the completion of your service. Please ask the clerk who is dismissing you for the form.
